Mobility/Community Access

Long term goal 

By the next annual review, Annie will move safely within the school (classroom, hallway, playground, cafeteria) using appropriate supports, with reduced falls, in 80% of observed transitions across 3 consecutive data days.

Short term goals

 Within 9 weeks, Annie will ambulate on level indoor surfaces between classroom locations (ex: center to center, classroom to restroom) with close supervision and no more than one loss of balance per transition in 80% of opportunities across 3 consecutive data days.

 Within 18 weeks, with adult guidance, Annie will stop, hold a staff hand or railing, and then resume walking when approaching curbs, stairs, or uneven surfaces in 80% of opportunities across 3 consecutive data days.

 Within 27 weeks, during school or short community-based instruction outings, Annie will complete brief walking tasks (ex: from bus to classroom, from classroom to cafeteria) with at least one scheduled rest break as indicated by her or staff in 80% of opportunities across 3 consecutive data days.

Assistive technology

Gait or mobility aids (if PT recommends): Low-profile pediatric walkers, gait trainers, or strollers/transport chairs for longer community distances when fatigue and safety are concerns
